Job Summary
L.I.F.E. has in place CHIO/Transportation as a member of the client’s support team who works with the supervision of the Dept. Supervisor, to ensure L.I.F.E.’s standards are met regarding client safety, needs. CHIO/Transportation is responsible for the care, safety, and observation of clients; while the client are in their care, and also, but not limited to reporting and documenting observations and assistance performed.
Duties and Responsibilities include, but are not limited to
- Provide services for client as directed by the Individual’s Support Plan (ISP), Behavior Support Plan (BSP), QIDP, and Nursing
- Assist client with personal care:
- Transporting client to, and from Day/Night activities
- Reporting all client safety/health issues or concerns
- Observe client’s physical, mental and emotional status
- Report any change to House Supervisor, Dept. Supervisor
- Document observed changes
- Complete all daily CHIO/Transportation documentation by the end of the shift
- Assist in the administration of medication that are ordinarily self-administered
- Maintain a clean and orderly client environment within my vehicle
- Follow principles of infection control and Standard Precautions
- Provide transport to medical appointments, outside activities and shopping
- Assist in providing a safe environment and promote quality client care through adherence to established policies, procedures and standards
- Able to lift/push over 75lbs.
Qualifications
Applicant Should
- Possess a high school diploma (GED acceptable) or demonstrate the ability to read and write adequately to complete required documentation and follow verbal and written instruction
- Have successfully completed the IFSSA/DDARS Waiver training module for attendant care or other applicable training from a qualified provider agency or have previous experience as an aide in a home health agency, hospital or long-term care facility or in serving persons with developmental disabilities
- Have CPR/First Aid certification
- Be in adequate physical health to perform job tasks required
- Be free from communicable diseases
- Have an interest in and empathy for persons with developmental disabilities
- Possess interpersonal skills necessary to work productively with individuals with developmental disabilities
- Have successfully completed orientation with provider agency and Direct Support Professional training
- Valid driver license
